अर्शोऽन्वितं भवति वर्त्म तु यत्तथाऽर्शः शुष्कं तथाऽर्बुदमथो पिडकाः सिराजाः | जालं सिराजमपि पञ्चविधं तथाऽर्म छेद्या भवन्ति सह पर्वणिकामयेन ||६||

sanskrit

"A disease associated with hemorrhoids (Arsha) leads to a condition called 'Vartma' (path) where the Vartma is dry, similar to the growth of a tumor (Arbuda) or pustules (Pidaka), veins (Sira), and the network of veins (Sira-jala). These five types of conditions, including the one associated with Arman (a specific growth), should be treated with incision (Cedya), as well as the disease known as Parvanika."

Diseases that should be treated with scarification (Lekhya) include those with the following characteristics: Utsangini (a type of swelling), Bahala-vartma (extensive path), Kardama-vartma (mud-like path), Shyava-vartma (dark-colored path), as well as the path that is bound (Baddha-vartma). Also included are the conditions with Klishta (degenerated) paths, those associated with Potaki (a kind of growth), Kumbhīkinī (a specific type of growth), and those involving Sharkara (sugar-like deposits).

The diseases that should be treated with excision (Bhedya) include those associated with Shleshmo-panaha (a type of obstruction), Lagana (adhesion), and Visa-vartma (path of poison), as well as Granthi (a swelling or knot), which is caused by a worm and known as Anjana (a type of infection). Also, excision is advised for the cases where the Sira (vein) is involved, as well as in cases of Akshi-paka (eye diseases due to inflammation) caused by both types of swelling and those which affect the vision by causing obstruction in the air channels (Vata).

"The diseases caused by Puyalasa (pus formation), Anilaviparyaya (disorders related to the air element), and Manthasanjna (disturbance of the mind) should be treated with venesection (Sira-vyadhana) as they can be alleviated through this method. In cases of dry Akshi-paka (eye diseases with dryness), Kapha-Pitta-vikridita-Dristi (impaired vision due to the combined aggravation of Kapha and Pitta), Amalakya (sourness), Shukra (impairment in reproductive fluid), and the use of Arjuna and Pishtaka medicines, these diseases are also treated with venesection."

"In cases of Aklinna-vartma (unmoistened path), Huta-bhug (path afflicted by fire or intense heat), Dvaja-darshi (vision affected by a flag or banner-like formation), and Shuktika-praklinna-vartma (path affected by mucus or discharge resembling a mollusk), as well as in diseases with the name Balasa, the authorities do not recommend surgical incisions. They assert that these conditions, when caused by an external influence or disease, should not be treated by instruments or surgical measures."
