सन्न्यस्तसञ्ज्ञो भृशदुश्चिकित्स्यो ज्ञेयस्तदा बुद्धिमता मनुष्यः | यथाऽऽमलोष्टं सलिले निषिक्तं समुद्धरेदाश्वविलीनमेव ||२१||

A person who has lost consciousness due to severe delusion should be regarded as extremely difficult to treat. An intelligent physician should treat such a case swiftly, just as one would immediately lift a lump of clay from water before it dissolves completely.

Similarly, in such cases, the physician should act quickly and treat the patient who is perspiring, nearing death, or in the grip of mortality. This can be done using sharp medicinal eye salves, unguents, inhalation of smoke, and by pricking the nails to stimulate consciousness.

The person should not be roused from unconsciousness through loud music, sweet words, or by shaking, rubbing, or applying pressure to secret points, as these methods will not restore consciousness if the patient does not respond. The use of soothing sounds, shaking, or deep breathing is also not recommended in such a case.

Once the person regains consciousness, they should be given emetics to clear the system, followed by a purified, light diet. They can then be treated with a combination of fruits, such as Triphala, Citraka, and Nagara (ginger), along with medicinal preparations made from ash or barley. For further treatment, a special mixture of clarified butter, especially old ghee, with sugar, should be given over the course of a month.
