पक्षाद्दशाहादथवाऽप्यकस्मात्तस्यार्धभेदं त्रितयाद्व्यवस्येत् | शङ्खाश्रितो वायुरुदीर्णवेगः कृतानुयात्रः कफपित्तरक्तैः ||१६||

Whether the pain occurs on one side or at any other time, its condition should be classified based on the involvement of the three doshas. The wind (Vayu) which has been disturbed by the doshas of Kapha, Pitta, and blood, moves with force and causes a pulsating, severe pain, resembling the flow of a conch-shaped air current.

रुजः सुतीव्राः प्रतनोति मूर्ध्नि विशेषतश्चापि हि शङ्खयोस्तु | सुकष्टमेनं खलु शङ्खकाख्यं महर्षयो वेदविदः पुराणाः ||१७||

The severe pain especially manifests in the head, and is especially intense in the region of the conch (ear). This very painful condition, known as Shamkhaka, is described by the great sages and those well-versed in the Vedas and Puranas.
