सिरासु पिडका जाता या न सिध्यन्ति भेषजैः । अर्मवन्मण्डलाग्रेण तासां छेदनमिष्यते ॥२१॥

The skilled physician should apply the instrument to the joints (Sandhous) after sweating them with the proper technique. He should then focus on the upper part and the three-quarters section, using a sharp implement for precise treatment.

छिन्द्यात्ततोऽर्धमग्रे, स्यादश्रुनाडी ह्यतोऽन्यथा । प्रतिसारणमत्रापि सैन्धवक्षौद्रमिष्यते ॥२४॥

The physician should cut the affected area at the front portion. If done incorrectly, it may cause a discharge of tears and damage to the nerves. In such cases, salt mixed with sugar should be used for the treatment.
