An ulcer that is to be managed (Yapya) should be treated with appropriate measures. If the patient adheres to the prescribed treatment, the ulcer will respond positively. However, if the treatment is not followed correctly or is interrupted, the ulcer will deteriorate, similar to how a building collapses if its support is removed.

Now, let's discuss the types of ulcers that are difficult to manage. These include: Ulcers that arise from the flesh and are accompanied by internal pus, pain, and have a condition similar to that of the intestines after a horse's death, or those that show hardened and protruded edges similar to a cow's horn, or those with severe blood loss and a delicate, thin appearance. Ulcers that have a texture resembling a worn-out cloth, with soft, translucent exudation, and are difficult to observe, or those that exude pus and marrow, having significant damage. Ulcers with a discharge of yellowish or blackish urine and stools, which are typically located in the abdominal region, or those associated with pain and swelling, and exhibit signs of an imbalance in bodily humors. Ulcers that are visible all over the body, have a bubbling appearance, or are associated with significant symptoms like cough and breathlessness, particularly when found in the head or throat area, or those showing a general weakness. Ulcers that are deep and show severe systemic symptoms, including signs of all three doshas (bodily humors), cough, and breathlessness. These types of ulcers are typically hard to treat and require careful management.
