द्विपलाः सलिलद्रोणे पक्त्वा पादावशेषिते । भार्गीपाठाढकीकुम्भनिकुम्भव्योषरोहिषैः ॥ २१ ॥

All these previous ingredients (each weighing two pala) are boiled together in one drona (12.288 liters) of water and reduced to one-fourth to prepare a decoction. To this decoction, a fine paste weighing one aksha (12 grams) each of bhargi, patha, adhaki, kumbha (trivrit), nikumbha (danti), vyosha (black pepper, long pepper, and ginger), and rohisha is added.

मूर्वाभूतिकभूनिम्बश्रेयसीशारिवाद्वयैः । मदयन्त्यग्निनिचुलैरक्षांशैः सर्पिषः पचेत् ॥ २२ ॥

Along with the previous paste ingredients, add one aksha (12 grams) each of murva, bhutika, bhunimba, shreyasi, the two types of sariva (shveta and krishna), madayanti, agni (chitraka), and nichula. This combined paste is then cooked with one prastha (768 grams) of ghee.

प्रस्थं तद्वद्द्रवैः पूर्वैः पञ्चगव्यमिदं महत् । ज्वरापस्मारजठरभगन्दरहरं परम् ॥ २३ ॥

The one prastha of ghee is systematically cooked with the decoction, the medicinal paste, and equal parts of the panchagavyas (liquid components: cow's dung juice, milk, curd, and urine). This formulation is known as Mahapanchagavya Ghrita. It is an excellent remedy to cure fever, epilepsy (apasmara), abdominal enlargements (udara), and fistula-in-ano.

शोफार्शःकामलापाण्डुगुल्मकासग्रहापहम् । ब्राह्मीरसवचाकुष्ठशङ्खपुष्पीशृतं घृतम् ॥२४॥

It also effectively destroys edema (shotha), hemorrhoids (arsha), jaundice (kamala), anemia (pandu), abdominal tumors (gulma), cough (kasa), and afflictions caused by evil spirits or planetary seizures (graha). Ghee cooked and processed with the fresh juice of brahmi (Bacopa monnieri), and the fine paste of vacha (Acorus calamus), kushtha (Saussurea lappa), and shankhapushpi (Convolvulus pluricaulis),

पुराणं मेध्यमुन्मादालक्ष्म्यपस्मारपाप्मजित् । तैलप्रस्थं घृतप्रस्थं जीवनीयैः पलोन्मितैः ॥२५॥

especially when using aged or old ghee (purana ghrita), acts as a potent intellect-promoter (medhya). It effectively cures insanity (unmada), inauspiciousness/misfortune (alakshmi), epilepsy (apasmara), and disorders born out of past misdeeds or sins (papma).
