तत्र स्निग्धस्विन्नमातुरं यथादोषप्रत्यनीकं द्रवप्रायमन्नं भुक्तवन्तं यवागूं पीतवन्तं वा यथाकालमुपस्थाप्यासीनं स्थितं वा प्राणानबाधमानो वस्त्रपट्टचर्मान्तर्वल्कललतानामन्यतमेन यन्त्रयित्वा नातिगाढं नातिशिथिलं शरीरप्रदेशमासाद्य प्राप्तं शस्त्रमादाय सिरां विध्येत् ||६||

In this context, after making the patient undergo oil massage and fomentation, and ensuring they have consumed a liquid-based diet appropriate to their dosha, or having consumed barley gruel, the patient should be positioned either seated or standing at the proper time. Without causing difficulty in breathing, the concerned body part should be securely, but neither too tightly nor too loosely, tied with a bandage made of cloth, leather, or bark of a tree. Then, after making the necessary preparations and using the appropriate surgical instrument, the vein (Sira) should be punctured.

The puncturing of veins (Sira) should never be performed when it is extremely cold, extremely hot, during strong winds, or when the sky is overcast, nor should it be performed on a person who is not in good health.

In this procedure, the patient whose veins are to be punctured should be seated facing the sun, on a seat that is slightly elevated. The knees should be bent and placed on the thighs, while both elbows should be placed over the junction of the two shoulder joints. The hands should be clasped together, with the thumbs hidden within the fists. A piece of cloth should be wrapped around the neck of the patient by another person standing behind him, who should hold the two ends of the cloth with his left hand while holding his breath. The person assisting the procedure should apply pressure on the patient's back to facilitate blood flow. This is the procedure for puncturing the veins of the upper parts of the body, except for the internal veins. For puncturing the veins of the feet, the patient should stand firmly on one foot, while the other foot should be slightly raised. The area where the vein is to be punctured, just below the knee joint, should be wrapped with a cloth and pressed firmly. The location of the vein, four fingers above the ankle, should be marked and punctured. For puncturing the hand veins, the arms should be placed correctly in the seated position, and the hand should be wrapped and punctured following the previous procedure. For veins of the back, hips, and shoulders, the patient should sit with his knees and elbows slightly bent, and his head tilted downward. For the abdomen and chest, the patient should be seated with his chest expanded, head slightly raised, body extended, and supported by both arms. For the groin, the patient should not bend the groin area while the vein is being punctured. For the mouth and palate, the tongue should be stretched upward or downward according to the need. For puncturing the gums and teeth roots, the mouth should be opened wide. This method of using instruments and pressure is determined by the condition of the body and the disease, and should be employed intelligently.

In other areas, it should be inserted half the depth of a barley grain or to the depth of a rice grain, with the tip of the instrument shaped like a rice grain.
