Then, after releasing the instrument by a barley grain in the left side, he should inspect it according to the evidence of the stone. Some say that this is done to ensure the effectiveness of the procedure; hence, it should not break or grind. Even a small powder, if established, can increase again; therefore, it should be provided with the entire anterior curvature. In women, the uterus is adjacent to the bladder, so the instrument should be dropped at that point; otherwise, it may cause a wound that leads to urinary leakage. For a man, the extraction of the urinary instrument can lead to urinary discharge, unless there is a stone wound. Even if there is a single broken bladder, it is not effective. A single bladder cannot be successfully treated if there is a stone wound; the single bladder caused by the stone wound will live. Due to the practice of the procedure, the instrument should be cut as prescribed; for it increases with the action of the surgeon. The extracted instrument should be immersed in warm water; otherwise, the bladder may not be filled. If full, it should be treated with milk, decoction of herbs, and flower nectar. Then, after releasing the instrument by a barley grain in the left side, he should inspect it according to the evidence of the stone. For cleansing the urinary tract, administer a mixture with jaggery. After extracting the stone, apply honey and ghee to the wound and give warm barley gruel (yavagu) processed with urine-cleansing medicines and ghee, twice daily for three nights. After three days, provide soft food with thickened milk and jaggery for ten days to help cleanse the blood and keep the wound moist. After ten days, treat with sour fruits and the broth of wild animals. Then for another ten days, apply oil or liquid fomentation (snehana), and wash the wound with decoction of Kshiravriksas (milky trees). Apply a paste of lodhra, licorice, manjistha, and prapaundarik to the wound. Oil or ghee, cooked with these herbs and turmeric, should be applied as a wound ointment. Treat stagnant blood with Uttarbasti (a specialized enema treatment). If after seven days the urinary path is not restored, cauterize the wound as per the prescribed method. Once the urinary path is restored, follow up with Uttarbasti, decoctions, and enemas using sweet and astringent medicines. If by chance, any semen-stone (shukrashmari) or gravel (sharkara) gets obstructed in the urinary tract, remove it surgically or by cutting open the tract. Once the wound is healed, the patient should avoid mounting animals like horses, elephants, or climbing trees for a year. Avoid swimming and consuming heavy food as well.
