मूढगर्भोदरार्शोऽश्मरीभगन्दरमुखरोगेष्वभुक्तवतः कर्म कुर्वीत ||१६||

In cases of impacted fetus (mūḍhagarbha), abdominal disorders (udara), hemorrhoids (arśaḥ), stones (aśmarī), fistula-in-ano (bhagandara), and other such diseases, surgical procedures (karma) should be performed on a patient who has fasted (abhuktavataḥ).

ततः शस्त्रमवचार्य,शीताभिरद्भिरातुरमाश्वास्य, समन्तात् परिपीड्याङ्गुल्या, व्रणमभिमृद्य(ज्य), प्रक्षाल्य कषायेण प्रोतेनोदकमादाय , तिलकल्कमधुसर्पिःप्रगाढामौषधयुक्तां नातिस्निग्धां नातिरूक्षां वर्तिं प्रणिदध्यात्; ततः कल्केनाच्छाद्य, घनां कवलिकां दत्त्वा, वस्त्रपट्टेन बध्नीयात्; वेदनारक्षोघ्नैर्धूपैर्धूपयेत्, रक्षोघ्नैश्च मन्त्रै रक्षां कुर्वीत ||१७||

After performing the surgery (śastram avacārya), the patient should be comforted by applying cold water (śītābhir adbhir). Then, press the area around the wound gently with fingers (samantāt paripīḍya aṅgulyā), massage the wound, and clean it with a decoction (kṣāyeṇa prakṣālya). After that, take clean water and apply a medicated wick (protena udakam ādāya... vartiṃ), which should be neither too greasy nor too dry, made from a paste of sesame, honey, and ghee (tilakalka-madhu-sarpiḥ). Cover the wound with a herbal paste (kalkena), place a dense cotton dressing (kavalikā), and bind it with a cloth (vastrapṭṭena badhnīyāt). Finally, fumigate the wound with pain-relieving and evil-repelling smoke (vedanā-rakṣoghnair dhūpaiḥ), and chant protective mantras (rakṣoghnair mantraiḥ) to safeguard the patient.

ततो गुग्गुल्वगुरुसर्जरसवचागौरसर्षपचूर्णैर्लवणनिम्बपत्रविमिश्रैराज्ययुक्तैर्धूपयेत् | आज्यशेषेण चास्य प्राणान् समालभेत ||१८||

After that, fumigate the wound with a mixture of powdered guggulu (Indian bdellium), aguru (eaglewood), sarjarasa (resin), vacā (sweet flag), gaura (a type of fragrant herb), mustard seeds, salt, and nimba (neem) leaves, all blended with ghee (ghṛta). After the fumigation, anoint the patient’s vital areas with the remaining ghee (ājyaśeṣeṇa prāṇān samālambhet). This procedure aims to ensure proper healing and protection of the patient.

After taking water from a water pot (udakumbha), sprinkle it while performing the protective rituals (rakṣākarmā). Now, we shall explain those protective rituals (tad vakṣyāmaḥ). This emphasizes the importance of rituals in ensuring the patient's safety during the healing process.

कृत्यानां प्रतिघातार्थं तथा रक्षोभयस्य च | रक्षाकर्म करिष्यामि ब्रह्मा तदनुमन्यताम् ||२०||

For the purpose of countering harmful actions (kṛtyānāṃ pratighātārthaṃ) and warding off the fear of malevolent spirits (rakṣobhayaṃ), I shall perform the protective ritual (rakṣākarm kariṣyāmi). May Brahmā approve of it (brahmā tad anumanyatām). This verse reflects the invocation of divine approval, specifically from Brahmā, before proceeding with the protective rituals, ensuring that both spiritual and practical harm is mitigated.
