चतुर्विधोऽर्शसां साधनोपायः | तद्यथा- भेषजं क्षारोऽग्निः शस्त्रमिति | तत्र, अचिरकालजातान्यल्पदोषलिङ्गोपद्रवाणि भेषजसाध्यानि, मृदुप्रसृतावगाढान्युच्छ्रितानि क्षारेण, कर्कशस्थिरपृथुकठिनान्यग्निना, तनुमूलान्युच्छ्रितानि क्लेदवन्ति च शस्त्रेण | तत्र भेषजसाध्यानामर्शसामदृश्यानां च भेषजं भवति, क्षाराग्निशस्त्रसाध्यानां तु विधानमुच्यमानमुपधारय ||३||

There are four types of remedies for hemorrhoids: medicinal, caustic, cautery, and surgical. In this context, remedies for hemorrhoids with slight symptoms that arise in a short time are treated with medicine. Those that are soft and protruding are treated with a caustic, while the hard, firm, and thick ones are treated with cautery. The thin, protruding ones that cause moisture are treated with surgical methods. Among the remedies, some are for hemorrhoids that are not visible, and others are for those that are visible; thus, the treatment through medicine is discussed, while the treatment through caustic, cautery, and surgery is also established.

Thereafter, the patient suffering from severe hemorrhoids, who is slimy and moist due to the increase of air, and who has consumed a small amount of warm, oily, and liquid food, should be seated on a clean surface in a place that is ordinary and appropriate. During an auspicious time, on a flat bed, or in the lap of another person, the patient should be seated in a relaxed manner, with a slightly raised neck and covered with a cloth or blanket. After securing the area with a device to prevent motion and with the assistance of an attendant, making the body steady and relaxed, one should apply medicated ghee to the anus and the device carefully. While the patient is experiencing a mild flow, the device should be held gently at the opening of the anus. After observing the progress, using a sharp instrument, one should carefully wipe with one of the two cloths, and then sprinkle caustic. After sprinkling, the device's opening should be covered with the hand, and for a moment, one should let it rest. Then, by wiping again and observing the strength of the caustic and the power of the disease, one should apply again. If the hemorrhoids are observed to have a mature appearance, the caustic should be washed away with water or with a solution made of honey, yogurt, or sour fruit juice. Next, a mixture of liquorice and honey should be applied, and the device should be raised. The patient should be seated with warm water sprinkled on them, and some say with cold water. Then, one should enter the treatment area and instruct the attendant; if there are remnants, they should be burned. This should be repeated over seven nights; each day, the procedure should be done with variations: starting from the right side, then the left, and then from the back, followed by the front.
