औषधमौषधसिद्धो वा स्नेहो नासिकाभ्यां दीयत इति नस्यम् | तद्द्विविधं शिरोविरेचनं, स्नेहनं च | तद्द्विविधमपि पञ्चधा | तद्यथा- नस्यं, शिरोविरेचनं, प्रतिमर्शो, अवपीडः, प्रधमनं च | तेषु नस्यं प्रधानं शिरोविरेचनं च; नस्यविकल्पः प्रतिमर्शः, शिरोविरेचनविकल्पोऽवपीडः प्रधमनं च; ततो नस्यशब्दः पञ्चधा नियमितः ||२१||

Nasal administration, or nasya, refers to the administration of medicine or oil through the nostrils. This method is of two types: head purgation (shirovirechana) and snehan (oleation). Each of these is further categorized into five forms: nasya, shirovirechana, pratimarsa, avapeeda, and pradhmana. Among these, nasya and shirovirechana are the principal methods. The variations of nasya include pratimarsa, while those of shirovirechana include avapeeda and pradhmana. Therefore, the term nasya is systematically categorized into five types.

In cases where oleation is prescribed for empty-headedness, strengthening the neck and shoulders, or for the purpose of enhancing vision, the term nasya is specifically applied. This treatment should be administered in conditions dominated by vata, targeting ailments affecting the head, teeth, hair, and facial hair, as well as severe pain in the ears, earaches, headaches, visual disturbances, nasal diseases, dryness of the throat, and the onset of weakness associated with vata and pitta doshas. The oleation must be performed with substances that are effective in alleviating vata and pitta.

शिरोविरेचनं श्लेष्मणाऽभिव्याप्ततालुकण्ठशिरसामरोचकशिरोगौरवशूलपीनसार्धावभेदक- कृमिप्रतिश्यायापस्मारगन्धाज्ञानेष्वन्येषुचोर्ध्वजत्रुगतेषु कफजेषु विकारेषु शिरोविरेचनद्रव्यैस्तत्सिद्धेन वा स्नेहेनेति ||२३||

Shirovirechana is indicated for conditions characterized by an abundance of mucus affecting the palate, throat, and head, as well as symptoms such as heaviness of the head, headaches, and conditions associated with a swollen head. It is also beneficial for various ailments related to the upper body, including issues like cough, sinusitis, epilepsy, loss of smell, and other conditions aggravated by kapha. This treatment should be carried out using substances that facilitate shirovirechana or with oleation that is effective in alleviating these symptoms.

Then, for a man who is to undergo shirovirechana (head purgation), after having given up urine and feces, at the right time, he should be seated in a clean place with a purified oral cavity, having his head and neck slightly elevated. He should be relaxed, with his arms and legs extended, and slightly delayed in action. With his eyes covered by cloth, and his left hand placed on the side of his nose, he should use his right hand to apply warm medicated oil—made of silver, gold, or copper, and mixed with various shells—using a cotton swab or similar instrument, ensuring that the oil flows smoothly, so that it does not enter the eyes.
