वातपित्तश्लेष्मसन्निपातागन्तुनिमित्ताः शतपोनकोष्ट्रग्रीवपरिस्राविशम्बूकावर्तोन्मार्गिणो यथासङ्ख्यं पञ्च भगन्दरा भवन्ति । ते तु भगगुदबस्तिप्रदेशदारणाच्च ‘भगन्दरा’ इत्युच्यन्ते । अभिन्नाः पिडकाः, भिन्नास्तु भगन्दराः ॥३॥
The types of Bhagandara (fistula in ano) arising due to derangement of Vayu, Pitta, Kaphah, Sannipatah (a simultaneous derangement of the three bodily Doshas), and extraneous causes are five in number: Shataponaka, Ushtragriva, Parisravi, Shamvukavarta, and Unmargi. They are termed 'Bhagandara' because they affect the region of the rectum, perineum, and bladder. The conditions in this area are initially known as Pidakas (non-suppurated pustules), but when they enter a stage of suppuration, they are referred to as Bhagandara.

In those who indulge in inappropriate foods, the deranged Vayu, when aggravated, solidifies and gives rise to a pustule within one or two fingers’ breadth from the anus. This pustule, which is red in color due to vitiated flesh and blood, causes specific pains such as pricking, and if untreated, it progresses to suppuration. The ulcer, due to its proximity to the bladder, becomes filled with pus through numerous sieve-like openings. These openings, continuously oozing thick, frothy discharge, cause the ulcer to feel as if it is being beaten, pierced, cut, or pricked. The anus cracks, and urine, fecal matter, and flatus are discharged through these openings. This type of fistula is referred to as Shataponakah.
