उपजिह्वां परिस्राव्य यवक्षारेण घर्षयेत् । कफघ्नैः शुण्डिका साध्या नस्यगण्डूषघर्षणैः ॥४६॥
Upajihva should be drained out by application of alkali prepared of Hordeum vulgare by rubbing. Śuṇḍikā should be treated by nasal medication, holding liquids in the mouth, and rubbing the paste of herbs which mitigate Kapha.

एर्वारुबीजप्रतिमं वृद्धायामसिराततम् । अग्रं निविष्टं जिह्वाया बडीशाद्यवलम्बितम् ॥ ४७ ॥
When it appears similar to ervāru (cucumber) seeds, has enlarged, is devoid of prominent blood vessels, and is situated near the back/root of the tongue, it should be supported/held using a Baḍiśa (hook/forceps) or similar instrument,

छेदयेन्मण्डलाग्रेण नात्यग्रे न च मूलतः । छेदेऽत्यसृक्क्षयान्मृत्युर्हीने व्याधिर्विवर्धते ॥ ४८ ॥
and excised with the Maṇḍalāgra Śastra (round-headed lancet/scalpel), neither too far towards the tip nor too close to the root. In case of deep cutting, death might occur from severe loss of blood; if cut inadequately, the disease increases (worsens).

मरिचातिविषापाठावचाकुष्ठकुटन्नटैः । छिन्नायां सपटुक्षौद्रैर्घर्षणं कवल: पुनः ॥ ४९ ॥
After cutting, the site should be rubbed with the paste of marica – Piper nigrum, ativiṣā – Aconitum heterophyllum, pāṭhā – Cissampelos pareira, vacā – Acorus calamus, kuṣṭha – Saussurea lappa, kuṭannaṭa – Cyperus scariosus, and salt added with honey; gargling should be done again

कटुकातिविषापाठानिम्बरास्नावचाम्बुभिः । सङ्घाते पुप्पुटे कूर्मे विलिख्यैवं समाचरेत् ॥ ५० ॥
with the decoction of kaṭukā – Picrorhiza kurroa, ativiṣā – Aconitum heterophyllum, pāṭhā – Cissampelos pareira, nimba – Azadirachta indica, rāsnā – Pluchea lanceolata, and vacā – Acorus calamus. Talu sanghata, Talupupputa and Kurma should be treated by scraping in the same manner.
