जरायुमात्रप्रच्छन्ने रविरश्म्यवभासिते । घृतस्य निश्चलं सम्यगलक्तकरसाङ्किते ॥ ३१ ॥

The area should be thin and translucent like a fetal membrane (Jarayu) when viewed against sunlight. The exact spot should be marked with the juice of Alaktaka (lac), where a drop of solid ghee stays steady without sliding.

विध्येद्दैवकृते च्छिद्रे सकृदेवर्जु लाघवात् । नोर्ध्वं न पार्श्वतो नाधः सिरास्तत्र हि संश्रिताः ॥ ३२ ॥

The physician should pierce exactly at this naturally indicated spot (Daivakrita chhidra) only once, straight, and with swift lightness. It must not be done too high, too low, or to the sides, because vital veins (siras) are situated around it.

कालिकामर्मरीरक्तास्तद्व्यधाद्रागरुग्ज्वराः । सशोफदाहसंरम्भमन्यास्तम्भापतानकाः ॥ ३३ ॥

These veins are named Kalika, Marmari, and Rakta; puncturing them causes redness, severe pain, fever, swelling, burning sensation, local inflammation, stiffness of the neck (Manyastambha), and convulsions or tetanus (Apatanaka).

तेषां यथामयं कुर्याद्विभज्याशु चिकित्सितम् । स्थाने व्यधान्न रुधिरं न रुग्रागादिसम्भवः ॥ ३४ ॥

If these complications arise, prompt medical treatment must be administered based on the specific condition. Conversely, piercing accurately at the correct site causes neither bleeding nor the appearance of pain, redness, or other complications.

स्नेहाक्तं सूच्यनुस्यूतं सूत्रं चानु निधापयेत् । आमतैलेन सिञ्चेच्च बहलां तद्वदारया ॥ ३५ ॥

After piercing, a thread smeared with oil or ghee should be passed through the hole following the path of the needle. The site should then be bathed with raw (uncooked) oil. Similarly, if the earlobe is thick, it should be pierced using a thicker needle (Ara).
