सुरादिमूत्रवान् बस्तिः सप्राक्पेष्यस्तमानयेत्। युक्तोऽल्पवीर्यो दोषाढ्ये रूक्षे क्रूराशयेऽथवा॥६॥
This decoction is blended with surā (fermented liquor), cow's urine (mūtra), and the therapeutic pastes (peṣya) mentioned previously to effectively draw out and evacuate the stagnant fluids and accumulated doṣas. When an enema of weak potency (alpa-vīryaḥ) is administered to a patient who has a massive accumulation of morbid doṣas (doṣāḍhye), excessive internal dryness (rūkṣe), or an extremely rigid bowel tract (krūra-āśaye), the fluid becomes overwhelmed and enveloped by the doṣas (doṣa-āvṛtaḥ).

Consequently, its onward passage is blocked (ruddha-mārgaḥ), which in turn severely obstructs the downward movement of Vata (rundhyāt samīraṇam). This displaced Vata, now moving in abnormal or upward directions (vimārgaḥ anilaḥ), produces acute abdominal distension and flatulence (ādhmānam) along with agonizing pain in the vital organs (marma-pīḍanam).

This diverted Vata further induces a severe burning sensation in the rectum and digestive tract (vidāhaṃ guda-koṣṭhasya), acute pain in the scrotum (muṣka) and groin areas (vaṅkṣaṇa-vedanām), and constricts or grips the cardiac region with sharp, piercing pains (ruṇaddhi hṛdayaṃ śūlaiḥ) as it violently darts back and forth through the internal channels (itaś-cetaś-ca dhāvati).

To counter this complication, the patient's body must first be thoroughly lubricated with medicated oil and subjected to sudation therapy (svabhyakta-svinna-gātrasya). Following this, a strong medicinal rectal suppository (vartim) should be introduced. Additionally, a corrective decoction enema using the bilva group of herbs (as described in verse 5) must be prepared and sharply enhanced by blending in the paste of pīlu (Salvadora persica), sarṣapa (mustard seeds), and cow's urine (mūtra-vān).
