बहुदोषस्य रूक्षस्य मन्दाग्नेरल्पमौषधम् । सोदावर्तस्य चोत्क्लेश्य दोषान् मार्गान्निरुध्य तैः ॥ ११ ॥

If a small quantity of medicine (alpam-auṣadham) is administered to an individual who has a massive accumulation of morbid doshas (bahu-doṣasya), who is dry (rūkṣasya), who has weak digestive fire (mandāgneh), or who suffers from udāvarta (upward movement of Vata), it merely agitates the doshas. These agitated doshas then block the internal channels (mārgān-nirudhya).

भृशमाध्मापयेन्नाभिं पृष्ठपार्श्वशिरोरुजम् । श्वासं विण्मूत्रवातानां सङ्गं कुर्याच्च दारुणम् ॥ १२ ॥

This blockage causes severe distension around the umbilicus (ādhmāpayen-nābhiṃ), intense pain in the back, flanks, and head (pṛṣṭha-pārśva-śirorujam), dyspnea (śvāsa), and a severe, painful retention (saṅga) of feces, urine, and flatus.

अभ्यङ्गस्वेदवर्त्यादि सनिरूहानुवासनम् । उदावर्तहरम्सर्वं कर्माध्मातस्य शस्यते ॥ १३ ॥

For a patient suffering from this severe abdominal distension (ādhmāta), oil massage (abhyaṅga), sudation (sweda), medicated rectal suppositories (varti), along with decoction enemas (nirūha) and oil enemas (anuvāsana), and all other treatments that mitigate udāvarta are highly recommended.

पञ्चमूलयवक्षारवचाभूतिकसैन्धवैः । यवागूः सुकृता शूलविबन्धानाहनाशनी ॥ १४ ॥

A well-prepared medicinal gruel (yavāgū) processed with Pañcamūla, yavakṣāra (alkali of barley), vacā (Acorus calamus), bhūtīka (Trachyspermum ammi), and saindhava (rock salt) effectively destroys colic pain (śūla), constipation/obstruction (vibandha), and abdominal distension (ānāha).

पिप्पलीदाडिमक्षारहिङ्गुशुण्ठ्यम्लवेतसान् । ससैन्धवान् पिबेन्मद्यैः सर्पिषोष्णोदकेन वा ॥ १५ ॥

A powder comprising pippalī (Piper longum), dāḍima (Punica granatum), kṣāra (barley alkali), hiṅgu (Ferula foetida), śuṇṭhī (Zingiber officinale), and amlavetasa (Garcinia pedunculata), along with saindhava (rock salt), should be consumed with wine (madya), ghee (sarpis), or warm water.
