After carefully examining the Bhagandara, whether ancient or modern, the surgeon should raise the Eshani (indicator) to identify the fistula's opening, and then use a surgical instrument to make an incision. In cases where the mouth of the fistula is directed inward, the instrument should be properly placed to address the flow of pus. The surgery may involve cauterization with fire or an alkali; this is a general approach applicable to all types of this condition.
