धूमैर्विरेकैः शिरसः पूर्वोक्तैर्गुल्मभेदनैः । तप्तायोहेमलवणपाषाणादिप्रपीडनैः ॥ ३१ ॥
Similarly, the condition should be managed with medicated fumigation (dhūma), purgative head-evacuations (śiro-virecana/nasya), and the tumor-breaking measures previously detailed for abdominal tumors (gulma). Finally, localized application of heat or compression using heated iron plates, gold, salts, or stones should be judiciously applied to open and resolve the swelling.

then its cauterization (dāha) utilizing chemical alkalies (kṣāra), red-hot iron arrows (śara), or a heated rod of gold (hema) is highly beneficial. Alternatively, it should be brought to suppuration using herbs that promote pus formation (pākibhiḥ), after which it must be surgically incised (pāṭayitvā) and completely extirpated (uddhavet).

The blood which is vitiated and coming up from the tumor should be let out in large quantity; after removing the blood, medicines which mitigate Vata and Kapha should be administered.

When the tumor becomes excessively moist and discharging due to the effects of cauterization or suppuration, it should be treated along the lines of management specified for external and internal wounds (vraṇa). For healing such a wound, an oil prepared and medicated with dārvī (Berberis aristata), viḍaṅga (Embelia ribes), and kampillaka (Mallotus philippensis) is highly beneficial.
