आदावेव विसर्पेषु हितं लङ्घनरूक्षणम् । रक्तावसेको वमनं विरेकः स्नेहनं न तु ॥ १ ॥
In herpes it is beneficial to administer langhana (thinning the body with fasting etc.), ruksana (causing dryness)rakta avaseko – blood-letting,vamanaṃ – emesis, virekaḥ – and purgation therapies in the beginning of treatment itself, but not snehana (oleation therapy).

Emesis done by using Randia dumetorum together with Glycyrrhiza glabra, Randia spinosa and indrayava – seeds of Holarrhena antidysenterica mixed with Trichosanthes dioica, Piper longum ,leaves of Azadirachta indica destroys visarpa.

For purgation therapy, the powder of trivṛt (Operculina turpethum) should be administered mixed with the juice or decoction of trāyantī (Gentiana kurroa), drākṣā (Vitis vinifera), or triphalā (Terminalia chebula, Terminalia bellerica, and Emblica officinalis); alternatively, it may be given along with milk or medicated ghee.

This cleansing line of treatment (virecana) should be specifically employed when the morbid doshas are densely accumulated and lodged inside the koṣṭha (alimentary tract/viscera). For a patient who is unfit for purificatory therapies (biocleansing/śodhana) and when the morbid doshas are mild, palliative therapy (śamana) is ideal. or a decoction of mustā (Cyperus rotundus), nimba (Azadirachta indica), and paṭola (Trichosanthes dioica); or the herbs of the Paṭolādi gaṇa (group of drugs); or śārivā (Hemidesmus indicus), āmalakī (Emblica officinalis), uśīra (Vetiveria zizanioides), and mustā boiled together in water should be administered.
