यूषै रसैर्वा मन्दाम्ललवणैरेधितानलम् । सोदावर्तं पुनः स्निग्धस्विन्नमास्थापयेत्ततः ॥ ५६ ॥
The digestive fire having been augmented by the use of vegatable soups (yūṣa) or meat soups (rasa) possessing mild sour and salt tastes, if upward movement of vāta (udāvarta) still persists, the patient should again be subjected to oleation and sudation, and then administered an asthāpana enema (decoction enema).

This should be done using a Daśamūla enema processed with herbs of strong, downward-evacuating action (tīkṣṇa-adhobhāga). For oil enemas (anuvāsana), a mixture of sesame oil (tila taila) and castor oil (urubūka taila) boiled with vāta-mitigating and sour substances should be used.

When there is throbbing (sphuraṇa), convulsions (ākṣepa), or pain in the joints, bones, flanks, back, and sacral region (trika), and the patient presents with dryness, obstruction of feces and flatus, but has a keen digestive fire (dīptāgni), an anuvāsana (oil) enema should be administered.

For a patient who is unfit for purgation therapy (avirecya), palliative treatments like enemas, milk, and ghee (śamana vasti-kṣīra-ghṛta) should be administered. In cases of abdominal enlargement caused by Pitta (pittodara), if the patient is strong, they should first be given oleation therapy using ghee processed with herbs of sweet taste (svādu).

Next, they should be given purgative therapy with ghee boiled with śyāmā – Ipomoea turpethum, tribhaṇḍī – Operculina turpethum, and triphalā. Following this, a nirūha (decoction) enema enriched with sugar, honey, and ghee is highly beneficial.
