नवज्वरे मलस्तम्भात्कषायो विषमज्वरम् । कुरुतेऽरुचिहृल्लासहिध्माध्मानादिकानपि ॥ ४१ ॥

even though these are mitigators of Pitta and Slesman (Kapha), astringent taste by producing obstruction to expulsion of Malas (Doshas/ feces), gives rise to intermittent fevers, and also causes loss of taste (and appetite), nausea, hiccup, flatulence etc.

सप्ताहादौषधं केचिदाहुरन्ये दशाहतः । केचिल्लघ्वन्नभुक्तस्य योज्यमामोल्बणे न तु ॥ ४२ ॥

Some authorities say that medicines (antipyretic) should be administered after seven days; others say, after ten days, some others say that it can be given when the patient is taking light foods. It should not be administered when there is increased production of Ama.

तीव्रज्वरपरीतस्य दोषवेगोदये यतः । दोषेऽथवातिनिचिते तन्द्रास्तैमित्यकारिणि ॥ ४३ ॥

When the patient is having very high fever, when the movement of Doshas have commenced, when there is great accumulation of Doshas (and also of Ama) producing stupor and inactivity,

अपच्यमानं भैषज्यं भूयो ज्वलयति ज्वरम् । मृदुर्ज्वरो लघुर्देहश्चलिताश्च मला यदा ॥ ४४ ॥

the medicine, if administered, not undergoing digestion, leads to greater increase of fever. When the fever is mild, there is feeling of lightness of the body, and normal movement of wastes (faeces, urine, flatus), then, medicines can be administered even though the fever is of recent onset.
