Shiva Purana
वेधा विधाता धाता च स्रष्टा हर्त्ता चतुर्मुखः ॥ कैलासशिखरावासी सर्वावासी सदागति ॥ १०९ ॥
He is the four-faced Vedha, Vidhata, Dhata, Creator and Destroyer. He resides on the peak of Kailas and is always there for all.

vedhA vidhAtA dhAtA ca sraSTA harttA caturmukhaH ॥ kailAsazikharAvAsI sarvAvAsI sadAgati ॥ 109 ॥
