यन्त्रे प्रविष्टं दुर्नाम प्लोतगुण्ठितयाऽनु च । शलाकयोत्पीड्य भिषग्यथोक्तविधिना दहेत् ॥ ६ ॥

yantre praviSTaM durnAma plotaguNThitayA'nu ca । zalAkayotpIDya bhiSagyathoktavidhinA dahet ॥ 6 ॥
hk transliteration by Sanscriptक्षारेणैवार्द्रमितरत् क्षारेण ज्वलनेन वा । महद्वा बलिनश्छित्त्वा वीतयन्त्रमथातुरम् ॥ ७ ॥
only if it (piles) is of the moist kind (bleeding piles) and the other kind (non-bleeding) with either caustic Alkali or fire (touching a red hot metal probe etc.) or excising (with sharp instrument) if the pile mass is big, especially in strong patients.

kSAreNaivArdramitarat kSAreNa jvalanena vA । mahadvA balinazchittvA vItayantramathAturam ॥ 7 ॥
hk transliteration by Sanscriptस्वभ्यक्तपायुजघनमवगाहे निधापयेत् । निर्वातमन्दिरस्थस्य ततोऽस्याचारमादिशेत् ॥ ८ ॥
Afterwards, the instrument is removed, the rectum and pelvis smeared with fats (ghee or oil) and the patient asked to sit in a tub (containing lukewarm or cold water (processed with herbs) kept in a chamber devoid of breeze (for sometime) and then instructed to adhere to the procedures (vide verses 26 and 27 of chapter 16 of Sutrasthana).

svabhyaktapAyujaghanamavagAhe nidhApayet । nirvAtamandirasthasya tato'syAcAramAdizet ॥ 8 ॥
hk transliteration by Sanscriptएकैकमिति सप्ताहात् सप्ताहात् समुपाचरेत् । प्राग्दक्षिणं ततो वाममर्शः पृष्ठाग्रजं ततः ॥ ९ ॥
The pile masses, one by one; should be treated week after week in this manner, that situated on the right side treated first and next, that situated on the left, and then the top on, when there are multiple masses.

ekaikamiti saptAhAt saptAhAt samupAcaret । prAgdakSiNaM tato vAmamarzaH pRSThAgrajaM tataH ॥ 9 ॥
hk transliteration by Sanscriptबह्वर्शसः सुदग्धस्य स्याद्वायोरनुलोमता । रुचिरन्नेऽग्निपटुता स्वास्थ्यं वर्णबलोदयः ॥ १० ॥
When the cauterization is proper, there is downward movement of Vata, desire for food, keenness of digestive fire, health and increase of strength and color /complexion.
